Defective Report


Purpose/Goal

The purpose of this SOP is to help you create the Defective Report that you will put out on the first Thursday of every month.

Step 1 - Use the Defective Log to create a report

Step 1 - Use the Defective Log to create a report


 

BEFORE YOU START: You will want the defective log open. 

  1. Open the defective log and filter dates to match the month of the report you are making it for.

    1. If there are no defective complaints for the month, just move on to expired. If there are no expired complaints, no report needs to be created, and you do not need to move on to the next steps. 

  2. Break down each complaint by product and then by flavor if necessary. Please list the number of complaints and their lot numbers, if applicable.    

    1. Please include expired products from the expired tab as well. 

    2. For expired units, please also include the order number and where the product shipped from (AMZ or Shipbob)

      1. Please include the warehouse the product shipped from in Shipbob; you can find the warehouse under shipping details on the order.  


BEFORE YOU MOVE ON:

Here is an example of what the report will look like: 


“Here are our defective complaints for Dec 3rd - Jan 5th. I have listed each complaint below! The products for which we received complaints are Blast, Brawn, and Build.

  • Blast- All complaints were for clumping

    • Blueberry Lemonade 2 complaints (Lot# 2203003 & 2201001)

    • Mango Peach 2 complaints (Lot#46679)

  • Brawn

    • Chocolate 2 complaints, one for a missing scoop and the other customer had two scoops (Lot# 46555

    • Banana 1 complaint for a broken seal (Lot#46501)

    • Tiramisu 2 complaints for being expired on 02/12/24, shipped from Shipbob warehouse Rancho Dominguez 

  • Build

    • Lemon Lime 2 complaints for a damaged or broken seal (Lot#47284)

Please let me know if there are any questions, and I will put the following report out on Feb 2nd.”

 

 

 

 

Step 2 - Put in the appropriate Slack Channel


 


  1. Put report in ops_osl_customer-service and tag Charlotte Bah & Kim S.
